APPLICATION FORM FOR CITIES – SMS PILOT PROJECT

This application form is aimed at expressing your interest in participating in the Standardized Monitoring System (SMS) pilot project.

We kindly ask you to answer to the following questions to provide some information about the City/Municipality you represent. After the reception of this form, our staff will contact you to provide more information about the following steps to participate in the project.

What City/Municipality do you represent?

How is the City/Municipality you represent committed with human rights ? (e.g. having declared it a human rights city, being part of cities’ networks, having policies/ municipal bodies/reporting activities dedicated to human rights , etc)

Does the City/Municipality you represent already monitor and report on human rights indicators?

If yes, do you have any good practice to share in this sense ? Where can these reports be found ?

Who will be the contact person and the institution responsible for the participation of the City/Municipality you represent in the SMS pilot project ?
